Date: Not dated, but the style appears to be from the 1960s or thereabouts.

Condition: Good, I'd say. The cover is a bit soiled and the striker strip shows at least some use. There's some damage to the striker strip area due to the removal of the staple that held the matches. There's also an additional pinhole which passes through the striker strip and both sides of the cover. (Please see scans.)
