Date: Not dated, but that old-style phone number probably puts it in the 1950s (or perhaps early 1960s)

Condition: Good, I'd say. The cover is only lightly soiled and the striker strip doesn't appear to show any evidence of actual use. However, the striker strip does have a bit of damage due to the removal of the staple that held the matches. There's also an additional pinhole which passes through the striker strip and both sides of the cover. (Please see scans.)
