It has a green control pad, a yellow kick button, and a yellow tackle/pass button.

It comes with the electronic handheld game including battery compartment cover.
It requires 4 "AA" batteries (not included).
A quick test was done and it worked fine.
The sound was clear and everything appeared to be in working order.
We did not play an actual game.
There is no original box or instructions.
