Wheat, the staff of life.
It looks like this plaque was made by taking impressions of the wheat heads, then adding a few details.
The tag says it's "made of clay from the Smoky Hill River banks near Kanopolis, KS."
(How appropriate - wheat from Kansas.)
The clay is beautiful. Not a white clay, but not a red like terra cotta, either.
Just a nice warm tannish color in the neutral family - would fit in with almost any color scheme.
There is a signature, looks like "INGH BO".
It comes with a brown cord for hanging, and measures about 7 1/4" tall by 3 1/2" wide.
There are no chips, cracks, or any other wear or damage on this beautiful piece.
